Housing Stabilization Advocate

Assist formerly homeless individuals and some families become independent and successful by assisting in obtaining permanent housing and providing follow-up case management services.
Help participants obtain and maintain mainstream benefits (e.g. food stamps, SSI, MassHealth)
Facilitate participants’ transition from homeless-based to community-based resources.
Work with participants to maintain their housing and prevent recurrent homelessness.
Assist participants in relocating as needed.
Aid participants in enrolling children in school and accessing all eligible resources needed by the family.
Establish and monitor mental health and substance abuse recovery plans.
Support participants in accessing detox or other appropriate substance abuse treatment.
Respond to participants who are in a mental health or other crisis (creating safety plans, accessing emergency services, suicide assessment and intervention, etc)
Travel throughout Greater Boston conducting home visits.
Assist with budgeting skills and other activities of daily living.
Participants include individuals and families with mental illness, long histories of substance abuse, and limited experience with independent living.
